Cadence launches AI‑powered Proactive Care Engine, bringing Advanced Primary Care Management to Medicare patients nationwide

Cadence gives health systems a scalable answer to workforce shortages and rising senior demand
Cadence, the remote care delivery system that has re‑engineered chronic disease management, today announced the national rollout of its AI‑powered Proactive Care Engine, enabling health systems to deliver Advanced Primary Care Management (APCM) to Medicare patients at scale. Addressing the full spectrum of seniors’ clinical, behavioral and social needs, the new system is setting the standard for primary care delivery.
Cadence supports more than 50,000 patients through remote chronic care programs across 18 leading health systems. With its Proactive Care Engine, those health systems can now extend the same infrastructure to primary care, moving from episodic visits to consistent, personalized support for every Medicare patient.

Cadence’s AI-powered Proactive Care Engine is live today with Lifepoint Health and Community Health Systems, with additional health systems launching later this year.

Proactive, AI‑powered care
Integrated directly into health system EHRs, the Proactive Care Engine scans each patient’s chart to detect care gaps and recommend next‑best actions. Cadence is identifying thousands of preventive care opportunities for patients – including incomplete vaccinations (e.g., Pneumovax), cancer screenings (e.g., colonoscopy) and chronic disease exams (e.g., diabetic eye and foot exams) – with a closure rate of nearly 30% for patients participating in the program for at least 90 days.

CMS introduced the APCM payment model in 2025 to reimburse proactive, team‑based primary care. Cadence offers leading health systems a turnkey service that improves outcomes while reducing total cost of care.
