Cadence confirms participation in CMS ACCESS Model, launching new AI-first care model for outcomes-based care

Cadence, the nation’s leading remote patient care system, confirmed its intent to participate in the CMS ACCESS Model, taking the next step toward scaling AI-powered, clinician-led chronic disease care across Medicare.
The new, AI-native program will build on Cadence’s demonstrated track record of driving measurable outcomes across entire populations. Since 2021, Cadence has partnered with more than 20 leading health systems to co-develop and deploy technology-enabled care models, serving over 130,000 patients while strengthening local physician relationships.
Under ACCESS, Cadence is launching a longitudinal care program specifically for patients not currently reached by existing comprehensive chronic disease management and prevention offerings. Working with health system Accountable Care Organizations, Clinically Integrated Networks, and affiliated physicians, Cadence will extend a new, AI-powered remote care model directly into community practices without adding financial risk or administrative burden for physicians.
Initial efforts will focus on rising-risk Medicare patients in the Cardio-Kidney-Metabolic and Early Cardio-Kidney-Metabolic tracks.
Peer-reviewed data demonstrate that patients in Cadence programs achieve meaningful clinical improvements, including a 70% increase in blood pressure control, a 27% reduction in hospital admissions, and a $1,302 per-patient annual reduction in total cost of care, with consistent results across rural and underserved communities. ACCESS reinforces that standard through measurable improvement and public reporting.
“ACCESS rewards what matters: real improvement in patient health,” said Chris Altchek, CEO & Founder of Cadence. “We are building the AI infrastructure for chronic disease care in America. We are laser focused on delivering better outcomes, strengthening local physician relationships through our health systems, and proving this model works on a national scale.”
As the first ACCESS cohort launches in July, Cadence looks forward to demonstrating measurable improvement for Medicare beneficiaries.
