What services does the program provide?
The program offers personalized support to help you stay on top of your health between doctor visits. How often will I speak with a care team member?
Your care team is available 24/7 to support your healthcare needs. The frequency of communication depends on your health goals and preferences. You may speak with your care team as often as needed to help you achieve your goals. We will build the care plan that works best for you.
How do I enroll?
Enrolling in the program is as easy as talking with your doctor. You may already be eligible for the program if you are enrolled in Medicare or Medicare Advantage. At your next appointment, ask your doctor if this program is right for you.
How do you communicate with my provider?
Our care team works closely with your healthcare provider to ensure seamless coordination of your care by communicating through your provider’s electronic medical record. We share updates on your progress, any concerns that arise, and insights gathered from our interactions with you to ensure your provider has a complete picture of your care.
How much does the program cost?
Advanced Primary Care Management is a covered benefit under Medicare and Medicare Advantage and many patients see little to no cost for the program. Cost varies based on whether a patient has met their deductible and their types of insurance coverage. The services Cadence provides are delivered under the supervision of your provider, and your provider bills your insurance directly for Advanced Primary Care Management. If you are considering enrolling in the program and have questions about how much it could cost, it is recommended to call your insurance company directly and share this billing code for an estimate: G0557 - APCM Level 2. Does Cadence offer any other programs?
Cadence offers remote patient monitoring services to patients with high blood pressure, type 2 diabetes, and/or heart conditions through certain hospital systems, clinics, and providers. Speak with your doctor’s office directly to learn whether this program is offered in your area.
