This patient joined the program without any chronic conditions — but still got value right away. We helped her get caught up on vaccinations, shared resources to address a home health concern, and built a plan to support her top goal: feeling better and losing weight.

When this patient shared they were struggling to get enough food, we stepped in to help. Our team worked directly with SNAP to get the process moving, removing barriers that had held them back for years and making access to care feel easier, not overwhelming.
