We pair patients with a dedicated clinical team, integrate deeply into health system EMRs and workflows, and use our Clinical Intelligence platform to monitor vitals, surface risk early, optimize medications, and close care gaps between visits. The result: patients engage with care 100x more than before Cadence, clinicians focus on judgment instead of administrative work, and Medicare saves $2M a week.
We operate as a full clinical care delivery organization, not a software vendor. Our clinicians work alongside health system partners, extending the reach of local primary care providers into patients' homes. We're now applying AI agents across these workflows – from alert review and medication titration to lifestyle coaching and care coordination – with clinicians always in control of clinical decisions.
The Role
We're hiring a Care Delivery Operations Lead to build and own the operating infrastructure that enables Cadence's clinical teams to deliver care at scale. You'll translate patient growth targets into workforce architecture, performance systems, and automation strategy - working across Clinical, Product, and Engineering to build a care delivery engine capable of supporting significant expansion without sacrificing quality or compliance. Most of what keeps 100,000+ patients healthier happens between visits; this role is responsible for making sure the systems behind that care never break under scale.
What You'll Do
- Build and maintain capacity planning and workforce forecasting models that translate patient growth and acuity targets into staffing architecture for RN workflows across Cadence programs.
- Define SLA standards, productivity benchmarks, and performance metrics in partnership with clinical leadership, then design the accountability structures that make them operational.
- Develop KPI dashboards and reporting infrastructure that give clinical and operational leaders real-time visibility into throughput, quality, and risk.
- Architect standardized triage workflows and documentation frameworks across care delivery operations, building the playbooks that scale into new markets and conditions.
- Partner with Product and Engineering to define automation strategy - identifying where workflow enhancements and AI-assisted tools can increase throughput while preserving clinical guardrails.
- Identify long-term operational risks before they surface as patient impact, and build mitigation strategies into the infrastructure rather than the response layer.

- Able to move between strategic architecture and execution detail - you can design a workforce model and then sit with the data until the numbers are right.
- Comfortable leading cross-functional initiatives across Clinical, Product, and Engineering without formal authority over any of them.
- Rigorous about performance governance - you build dashboards and reporting structures because you want accountability, not because someone asked for a metric.
- Grounded in the operational reality of care delivery - you understand that a broken triage workflow doesn't just miss an SLA; it affects a patient managing hypertension or heart failure between visits.
- Biased toward action - you tighten every process you touch, close every loop you open, and never drop a ball.
What You Need
- 7-10 years building and scaling operations in high-growth, complex environments; healthcare, RPM, telehealth, or value-based care experience is a strong differentiator.
- Demonstrated experience designing large-scale operating models, including workforce architecture, capacity planning, and SLA frameworks.
- Proven ability to build performance governance systems - KPI dashboards, reporting cadence, and accountability structures - that drive measurable outcomes.
- Experience leading cross-functional initiatives across Clinical, Product, Engineering, and Operations in regulated environments.
- Familiarity with nurse staffing models, triage operations, or chronic disease management programs is preferred.
- Background in process improvement methodologies such as Lean, Six Sigma, or comparable frameworks is preferred.
- Fluency with AI productivity tools (such as Gemini, Notion AI, or comparable platforms) and comfort applying them to structured workflows, reporting, and process documentation.
The anticipated base salary range for this role is $150,000-$180,000 annual base salary, which is based on role scope, level, and location. In addition to base compensation, this role may be eligible for incentive compensation as part of the overall total rewards package. This position is remote and based in the United States. Actual compensation will be determined based on factors including experience, skills, internal equity, and applicable law.
