Heart Failure

Heart failure is expected to increase by 50% over the next 10 years, and is a common cause of morbidity and mortality for individuals with cardiovascular disease.

Despite proven benefits and strong guideline recommendations for certain guideline directed medical therapies (GDMT), clinicians have a difficult time initiating patients on appropriate therapies at target doses. Using remote patient monitoring, Cadence can help increase the adherence to these life-saving therapies by using more frequent vitals data.

Type 2 Diabetes

Over 10% of Americans have been diagnosed with diabetes. People with diabetes are at increased risk for heart disease, such as heart attacks or strokes.

Education and self-management strategies have been very effective in helping to improve glycemic control for people with diabetes. With access to remote patient monitoring, education, and the clinical expertise of the Cadence team, people with diabetes can exercise tighter blood glucose control, and reduce their risk of diabetes-related complications.

Hypertension accounts for more cardiovascular disease death than any other modifiable cardiovascular disease risk factor.

Antihypertensive medication use helps reduce the risk of cardiovascular disease and all-cause mortality among adults with hypertension. With Cadence and remote patient monitoring, patients and clinicians can accurately assess blood pressure trends outside of a hospital or doctor's office to allow for more appropriate medication titration, and better blood pressure control.

Chronic Obstructive Pulmonary Disease

Chronic obstructive pulmonary disease has been a leading cause of death and hospitalizations nationwide for decades. As our population ages and individuals continue smoking, the burden of COPD on the healthcare system will remain.
