Cadence announces intended participation in ACCESS model as CMS advances outcomes-based chronic disease care

The newly announced ACCESS Model is a breakthrough for chronic-disease care. By tying payment to real outcomes, CMS is aligning incentives around what matters most – improving patient health – and signaling support for interventions backed by strong clinical evidence. Cadence has submitted its statement of intent to participate and looks forward to expanding Remote Patient Care (RPC) to thousands more Medicare beneficiaries.

Cadence’s RPC program already delivers the kind of measurable improvement ACCESS is designed to scale. Recent peer-reviewed data show consistent, meaningful gains in cardio-metabolic outcomes alongside reductions in total cost of care: 

  • JACC: Advances (July 2025): Cadence’s RPC program yielded meaningful blood pressure reductions, as the proportion of patients meeting the guideline target of <130/80 mmHg rose by 70%. Similar outcomes were seen among rural seniors, highlighting RPM's ability to reduce health disparities.
  • New England Journal of Medicine: Catalyst (Oct. 2025): A case study on the Cadence RPC program with Providence showed a 43% relative increase in blood pressure control, 107% increase in heart failure patients with reduced ejection fraction on all four GDMT pillars, and 300% increase in heart failure patients with preserved ejection fraction on all four GDMT pillars.
  • American Heart Association Scientific Sessions (Nov. 2025): Cadence's program for heart failure resulted in up to 3x improvement in GDMT uptake and monthly savings on average of $183 per patient driven primarily by a reduction in inpatient costs and hospital admissions.
  • Mayo Clinic Proceedings (Nov. 2025): Cadence’s RPC program led to annual total savings of $1,302 per patient, driven primarily by a 27% reduction in hospital admissions – specifically, reductions in hospitalizations for heart failure and stroke. This corresponds to 64 fewer hospitalizations per 1,000 patients at 12 months in the program.
  • JACC (Apr. 2024): Cadence’s RPC program enabled 43% of patients to reach their blood glucose goal (BG <154mg/dL) after 6 months in the program.
