The Role

We're hiring a Patient Engagement Specialist to own proactive outreach that keeps patients active and adherent in Cadence's remote monitoring program. This role sits between patients and their care team, driving appointment scheduling, supporting consistent monitoring, and re-engaging with patients. Success means patients stay connected to care between visits, not just during them.

Schedule: Standard working hours are Monday through Friday 9:00 AM to 5:30 PM on Central, Mountain, or Pacific Time.

What You'll Do

  • Own proactive outbound outreach to schedule patient appointments with Cadence clinicians and care team members.
  • Drive patient adherence to daily vital sign monitoring through consistent, personalized engagement.
  • Identify patients at risk of disengaging and execute re-engagement outreach to restore active participation.
  • Resolve patient questions, concerns, and objections related to appointments, monitoring, and program participation.
  • Coordinate with clinical and operational teams to ensure timely follow-up on flagged or at-risk patients.
  • Document every patient interaction and engagement outcome accurately and consistently in internal systems.
  • Strengthen the patient experience by delivering personalized, compassionate support across every touchpoint.

What You Need

  • 2+ years of experience in sales, customer service, patient outreach, or healthcare support roles.
  • Proven track record delivering service to patients or customers.
  • Excellent verbal and written communication skills, with the ability to build rapport and resolve objections.
  • Consistent track record of meeting performance metrics and engagement goals.
  • Proficiency with CRM platforms and Mac-based software tools in a fast-paced, metrics-driven environment.
  • Fluency with AI productivity tools (such as Gemini, Notion AI, or comparable platforms) and comfort applying them to structured workflows, reporting, and process documentation.

Compensation

The anticipated compensation range for this role is $23-$24/hr, which is based on role scope, level, and location. In addition to base compensation, this role is eligible for incentive compensation as part of the overall total rewards package with an OTE of $67K-$70K. This position is remote and based in the United States. Actual compensation will be determined based on factors including experience, skills, internal equity, and applicable law.
