How Cadence Connected Care helps
Devices delivered to the home for quick daily readings
Simple monitoring devices are shipped to your home at no cost. Most members spend just a few minutes per day using their device. Readings are securely transmitted for review.
Ongoing clinical review and 24/7 access
The Cadence Care Team reviews readings on an ongoing basis. Members can call or text 24 hours a day, 7 days a week.
Health guidance and support
Regular telephone appointments provide support with nutrition, movement, building healthy routines, and managing medications that support healthy blood sugar and blood pressure.
What to expect with Cadence Connected Care
Once your Cadence device arrives in the mail, follow the instructions to get started. You’ll be ready to take your first reading in just a few minutes. For help, you can use the setup videos on this page for guidance.
Start using your device to take regular readings. These readings are automatically sent to your Cadence Care Team, helping us to identify trends and provide personalized recommendations to support your health goals. If you leave the program, the devices are yours to keep.
Each time you use your device, we’ll text you your reading details. Every week, you’ll receive a summary email with trends, personalized recommendations, and a reminder of your next appointment.
In your first appointment, you’ll complete a medical intake, set a health goal to work toward, and determine follow-up appointments based on your initial readings. During regular check-ins, we'll review trends in your readings and provide guidance, coaching, and manage any medications that support healthy blood sugar and blood pressure.
