1. Set up your Cadence device at home

Once your Cadence device arrives in the mail, follow the instructions to get started. You’ll be ready to take your first reading in just a few minutes. For help, you can use the setup videos on this page for guidance.

2. Take your first 7 daily readings to unlock deeper insights

Start using your device to take regular readings. These readings are automatically sent to your Cadence Care Team, helping us to identify trends and provide personalized recommendations to support your health goals. If you leave the program, the devices are yours to keep.

3. Get updates and weekly insights from your Cadence Care Team

Each time you use your device, we’ll text you your reading details. Every week, you’ll receive a summary email with trends, personalized recommendations, and a reminder of your next appointment.

4. Attend appointments with your Cadence Care Team

In your first appointment, you’ll complete a medical intake, set a health goal to work toward, and determine follow-up appointments based on your initial readings. During regular check-ins, we'll review trends in your readings and provide guidance, coaching, and manage any medications that support healthy blood sugar and blood pressure.

The Cadence Care Team is made up of Registered Nurses, Nurse Practitioners, Medical Assistants, and Health Coaches who support eligible members nationwide and offer 24/7 access to care.

There is no additional cost for: devices, shipping, daily monitoring, 24/7 clinical access, or regular telephone appointments. How much time does it take?

Do participating Humana Medicare Advantage members need to visit an office to participate in Cadence Connected Care?

Will a doctor be informed?

If a participating Humana Medicare Advantage member would like their physician included, updates from Cadence Connected Care can be shared with the provider. The program does not require an existing physician relationship.
